Why can a recognized file still fail?

A container and the codecs inside it are separate. Browser decoder and encoder support varies by operating system and browser, so the tool checks the actual track rather than promising universal conversion.

Does processing change my original?

No. A separate result is created for preview or download; the selected source file is not modified.
